Please ensure you bring all required documents and medical information to avoid delays during your appointment.
D4, Taxi Council form, or employment medical paperwork.
Driving licence or valid photo ID.
Glasses or contact lenses if worn for driving.
Details of any medical conditions and regular medications.
You must bring proof of address (e.g. utility bill or phone bill) for taxi medicals only. Drivers on diabetes medication must provide 4 weeks of blood glucose readings (twice daily) with no hypos. Drivers on insulin cannot be assessed.
A medical summary is required for all taxi medicals across most councils. Some councils require full medical records. These can be requested from your GP reception.
In some cases, we may accept medical information from the NHS App depending on the council requirements.
